I agree that my participation in the production may be edited at the HomeTeamNS’ sole discretion and understand that I am to receive no compensation for this appearance. HomeTeamNS shall have complete ownership of the program. I consent to the use of my name, likeness, voice, and biographical material about me in connection with HomeTeamNS’ publicity and related institutional promotional purposes. The Producer may:
- Photograph me and record my voice and likeness for the purpose of the production mentioned above, whether by digital film or otherwise;
- Make copies of the photographs and recordings;
- Use my name and likeness for the purposes of education, promotion or advertising of the sale or trading in the photographs, recordings and any copies so made.
- Edit same at its discretion and to include it with the performance of others and with sound effects, special effects and music; to incorporate same into the program stated above or not; to use and to license others to use such recordings and photographs in any manner or media whatsoever, including without limitation unrestricted use for purposes of publicity, advertising and sales promotion; and to use my name, likeness, voice, biographic or other information concerning me in connection with the production above, commercial tie-ups, merchandising, and for any other purpose.
I further acknowledge that HomeTeamNS owns all rights to the results and proceeds of my services rendered in connection herewith. I expressly release HomeTeamNS from and against any and all claims which I have or may have for invasion of privacy, defamation or any other cause of action arising out of production, distribution, broadcast or exhibition of the program stated above.
I am entitled to withdraw my consent via email to dpo@hometeamns.sg, in respect to the collection and disclosure of my personal data at any point in time. Having received and verified my request, HomeTeamNS will purge my data within thirty (30) days from the date of request.
I understand that participation in this project is voluntary and that I may at any time discontinue my involvement; however, I understand that I cannot revoke the authorization I have provided with respect to any works produced prior to my withdrawal
